Registration Requirements.

In order to use all of our online services you are required to verify your identity through the use of a unique Personal Identification Number (PIN) given to you by our staff. 
You may register for our online portal with out a PIN.  If you register without a PIN you will be limited to "Request an Appointment" and "Pay and View Your Bill".

Please note that you will need to complete all 3 registration steps in order to access our portal:
After your first appointment is made your PIN will be sent to your secure message inbox and then you will be able to access all of our online features.

Fill out our new patient forms online and save time on your first visit. 
Items you will need inorder to complete the registration process. 
1:  Your current insurance card(s) (

2:  Your Medicare or Medicaid card (if applicable)

3:  Your primary physician's first and last name
4:  Your referring physicians first and last name
5:  A list of all your medications/allergies
6:  Your pharmacy name and telephone number
